(Attach certified copy of resolution duly signed by the President, Vice President,
Secretary, Treasurer or Joint Secretary and Executive Members, as applicable,
resolving to seek assistance under this scheme.)

(Attach letter/certificate from an elected Member of the State Bar Council from
the affected area attesting to the damage suffered. If no member is there, please
don’t fill the Annexure, and the Chairman of the BCI Advocates Welfare
Committee may decide.)
